004B Routine QOF work | b Weekly activities

This folder includes a set of searches that should be reviewed weekly to stay on top of QOF targets. Each search identifies patients with incomplete QOF reviews. 



๐Ÿงพ DQ | Asthma with incomplete review

This report lists patients with incomplete asthma reviews. It shows which required components are missing or incorrectly timed.
To meet the AST007 indicator, the following must be recorded:

  • Number of exacerbations

  • Written personalised asthma action plan

  • The report checks whether each component is coded within the valid QOF timeframe.

  • Patients may appear on multiple lines if elements were recorded at different times.

  • Blank cells indicate what is missing.

โœ… Once the review is fully completed and correctly coded, the patient will be included in AST007 and disappear from this report.


๐Ÿงพ DQ | COPD with incomplete review

This report highlights patients with incomplete COPD reviews.
To meet COPD010, the following should be reviewed and coded:

  • Number of exacerbations

  • MRC dyspnoea score

  • There are no timeframe rules for these elements โ€” the report simply pulls the latest available data.

  • Blank cells identify what is still missing.

โœ… When all components are recorded, the patient will be counted for COPD010 and removed from the report.


๐Ÿงพ DQ | Dementia review or care plan but not QOF code

This report lists patients who have had a dementia review or care planning activity, but without the correct QOF code (required for DEM004).

  • These cases typically need retrospective coding using a valid QOF-compatible review code.

  • This report supports efficient checking and correction without digging through clinical notes.

โœ… Once coded correctly, the patient will be included in DEM004 and disappear from the list.


๐Ÿงพ DQ | Heart failure with incomplete review

This report identifies patients whose heart failure review is incomplete.
To support achievement of HF007, it includes:

  • The review date

  • The latest medical review date

  • Missing elements from the QOF definition

This allows you to identify and code the missing parts without searching the full record.

โœ… Once all necessary codes are added, the patient will be counted for HF007 and no longer appear on this report.
