These searches help practices identify patients with Atrial Fibrillation where QOF-related actions may still be needed to meet indicators AF006 (stroke risk assessment) and AF008 (anticoagulation for high-risk patients). The searches are designed for use throughout the year and are especially valuable during the final months of QOF.
AF patients who have received one invitation for a atrial fibrillation care review. Sending a second invitation will allow these patients to be exception-reported if they do not attend.
AF patients who have received two invitations and not responded. These patients can now be exception-reported under AF006 if no further engagement occurs.
AF patients who have received one invitation to discuss anticoagulation. Sending a second invitation will allow these patients to be exception-reported if they do not attend.
AF patients who have received two invitations but are not on anticoagulation. These patients can now be exception-reported under AF008 if no further engagement occurs.
AF patients with no recorded CHAโDSโ-VASc score. These patients are not eligible for achievement under AF006 until the risk score is added.
Patients with raised stroke risk scores (CHAโDSโ-VASc โฅ2) who are on warfarin but do not have an exception code. Review these cases to code a justification for warfarin treatment (as opposed to being on a DOAC).
Patients with raised stroke risk scores (CHAโDSโ-VASc โฅ2) who are not on anticoagulation and do not have an exception code. Review these cases to either start treatment or code a valid exception.
