004B Routine QOF work | * Indicator Navigator | COPD

This folder supports practices in completing QOF work for COPD010, which focuses on the annual review of patients with Chronic Obstructive Pulmonary Disease. The searches help identify patients who still need a review, may be eligible for exception coding, or require follow-up due to incomplete or missed reviews in previous years.

Warning
The searches have been built to look at the contract year (1st April to 31st March), so it is recommended to run the search WITHOUT a relative run date.

๐Ÿ” [COPD010] One invitation sent

COPD patients who have received one invitation for an annual review. A second invitation is needed to support exception reporting if the patient does not attend.

๐Ÿ” [COPD010] Two invitations sent

COPD patients who have received two invitations but have not had a review. These patients are now eligible for exception coding under COPD010.


๐Ÿงพ DISENGAGED | COPD010 and didn't have a review last year

Patients with a COPD diagnosis who missed their annual review last year. Likely to be disengaged and clinically at risk โ€” useful for prioritising outreach.

๐Ÿงพ DQ | COPD with incomplete review

Patients who had a COPD review coded but did not meet full QOF criteria. This may be due to missing elements (e.g. MRC score, smoking status, CAT score). Review and coding may need to be completed or corrected.

๐Ÿงพ HIGH RISK | COPD010 no review last year and high risk

COPD patients who missed their review last year and are also marked as high risk (e.g. due to frequent exacerbations or poor control). Prioritise for proactive recall.

๐Ÿงพ PCA CHECK | COPD010 no review previous PCA

Patients who had a PCA code applied for COPD010 in a previous year (e.g. patient unsuitable or informed dissent). These should be reviewed to determine if a PCA is still appropriate this year, or if a review can now be completed.


๐Ÿงพ CLINICIAN | COPD - newly added to register

Patients who have been added to the COPD register for QOF v51, but they do not have a valid code for the previous register, but do have one or more codes from COPDDIAG_COD or COPDPROC_COD. Patients listed here are most likely going to have codes relating to COPD monitoring or invitations from the past 2 years, either used for other respiratory conditions such as bronchiectasis or accidentally used for recall. 

  1. Use the report to identify the affected codes and consider whether they need to be removed. All instances would need to be actioned to remove a patient from the register. 

๐Ÿงพ CLINICIAN | COPD - Removed from register

Patients who have been removed from the COPD register for QOF v51. They do have a code present in COPD_COD, but do not have any codes from COPDDIAG_COD or COPDPROC_COD. 

  • Use the report to identify the affected codes and consider whether other codes need to be added to restore patients to the COPD register.

The old COPD register allowed 56 diagnosis codes. The new register accepts approximately 11 of those codes plus a couple of other codes, and a further 13 procedure codes relating to monitoring and invitations. Many of the diagnosis codes which are no longer counted for the COPD register relate to emphysema and bronchitis.
