This folder supports achievement of DEM004, which relates to the completion of an annual dementia review. These searches help identify patients who need inviting, may have had care delivered without appropriate coding, or who had a PCA applied previously.
Dementia patients who have had one invitation for their annual review. A second invitation is required if you intend to exception-code a non-responder.
Patients who have had two invitations but no review completed. These are now eligible for exception coding under DEM004.
Patients where a review or care planning discussion has taken place, but the correct QOF code was not used. These are often correctable with retrospective coding.
Patients who had a PCA recorded last year (e.g. informed dissent or patient unsuitable) and no review this year. Review these cases to decide if a PCA should be re-applied or if a review can now be completed.
Patients who have had a contact or appointment this QOF year, but no dementia review is coded. May be an oversight or opportunity to code retrospectively if review was done but not recorded correctly.
