004B Routine QOF work | * Indicator Navigator | Hypertension

Hypertension

These searches support achievement of the HYP008 and HYP009 indicators, which focus on blood pressure control in patients with hypertension:

  • HYP008 – BP ≤140/90 mmHg in patients aged 79 or under

  • HYP009 – BP ≤150/90 mmHg in patients aged 80 and over

These searches help practices follow up on invitations, borderline results, high-risk cases, and exception coding opportunities.



🔍 [HYP008] One invitation sent

Patients aged ≤79 who have had one invitation for a hypertension review. Send a second invitation to enable exception coding under HYP008 if they do not attend. If the patient has had a raised blood pressure recorded this year, there must be at least two invitations after the most recent BP result.

🔍 [HYP008] Two invitations sent

Patients aged ≤79 with two invitations and no BP to target. Now eligible for exception coding under HYP008.

🔍 [HYP009] One invitation sent

Patients aged ≥80 who have received one invitation for review. Send a second invitation if the patient hasn’t attended to support future exception coding for HYP009. If the patient has had a raised blood pressure recorded this year, there must be at least two invitations after the most recent BP result.

🔍 [HYP009] Two invitations sent

Patients aged ≥80 with two invitations but no qualifying BP recorded. These are now eligible for exception coding under HYP009.


🧾 BORDERLINE | HYP008 would have achieved surgery BP target

Patients aged ≤79 whose latest BP reading was home or ambulatory that would have been within target if taken in surgeryHome/ambulatory thresholds are lower than surgery thresholds, so these patients may achieve HYP008 if a surgery reading is taken.

🧾 BORDERLINE | HYP009 would have achieved surgery BP target

Same as above, but for patients aged ≥80 under HYP009. Surgery BP could allow QOF achievement. These patients might meet HYP009 if they are invited in for a surgery-based reading.


🧾 CHECK BORDERLINE | HYP008 BP this year not to target NO previous PCA

Patients aged under 80 with BP not to target this year, but no history of PCAs recorded in the last 3 years. These cases may be borderline or newly raised — worth reviewing or repeating.

🧾 CHECK BORDERLINE | HYP009 BP this year not to target NO previous PCA

Patients aged 80+ with raised BP this yearbut no history of PCAs recorded in the last 3 years. These may reflect isolated events or measurement variation — worth rechecking.


🧾 HIGH RISK | HYP008 no BP reading this QOF year but latest was raised

Patients aged ≤79 with no BP this year, and last BP was above target. Likely to fail HYP008 if not reviewed before year-end.

🧾 HIGH RISK | HYP009 no BP reading this QOF year but latest was raised

Patients aged ≥80 with no BP this year and previously raised readings. At risk of failing HYP009 without a current check.

🧾 HIGH RISK | HYP no BP reading this QOF year but latest was raised

Catch-all list for hypertensive patients of any age with no BP this year and raised BP previously. Use to prioritise reviews and close gaps.


🧾 PCA CHECK | HYP008 BP this year not to target on 3+ antihypertensives

Patients under 80 who are not to BP target and already on 3 or more antihypertensives. Consider PCA for maximal tolerated therapy under HYP008.

🧾 PCA CHECK | HYP009 BP this year not to target on 3+ antihypertensives

Same as above, but for patients aged 80+ and HYP009. May qualify for PCA if further escalation is not appropriate.

🧾 PCA CHECK | HYP008 BP this year not to target previous PCA

Patients aged ≤79 who have not met BP target and had a previous PCA within the last 3 years. Review whether the exception remains appropriate.

🧾 PCA CHECK | HYP009 BP this year not to target previous PCA

Patients aged ≥80 who have not met BP target this year and had a previous PCA within the last 3 years. May need reapplying if clinical picture is unchanged.


🧾 QUICK WIN VALUE | HYP no BP reading this QOF year and latest was normal

Hypertensive patients with no BP recorded this year, but last BP was in target. A quick win if a BP check is done — likely to meet HYP008 or HYP009 with a single update.
