This folder supports achievement of NDH002, which relates to ensuring patients with a diagnosis of non-diabetic hyperglycaemia have an HbA1c or fasting glucose check annually. It also helps with exception coding and identifying patients likely to achieve with minimal input.

Patients eligible for NDH002 who have had one invitation for blood testing.
Action: Send a second invitation to enable QOF exception coding if the patient does not attend.
Patients with two invitations recorded who have not had a blood test. These can now be exception-coded for NDH002.
Patients with gestational diabetes who have been added to the NDH register, who have had an HbA1c recorded during the previous 2 contract years to today.
Patients with gestational diabetes who have been added to the NDH register, who have not had an HbA1c recorded during the previous 2 completed contract years to today.
Action: Consider how to contact these patients. They will require a blood test, but may be surprised about being invited. Some may not have had their HbA1c tested for years, so a softer approach may be appropriate to avoid confused patients contacting the Practice.
Patients who had a PCA applied in a previous year due to inability or refusal to complete an HbA1c test.
Action: Reassess whether the PCA should be re-applied or whether the patient is now able to complete testing.
Patients with NDH who are missing an HbA1c this QOF year, but likely to attend or previously engaged.
Action: These are ideal for opportunistic HbA1c testing โ a simple blood test will secure NDH002.
