004B Routine QOF work | * Indicator Navigator | Smoking

This folder supports achievement of the following smoking QOF indicators:

  • SMOK002 โ€“ patients with a long term condition (CHD, PAD, stroke/TIA, hypertension, diabetes, COPD, CKD, asthma, SMI) with smoking status recorded

  • SMOK004 โ€“ current smokers offered support this year or last year

These searches help manage invitations and PCA opportunities.

Warning
The searches have been built to look at the contract year (1st April to 31st March), so it is recommended to run the search WITHOUT a relative run date.

Patients eligible for SMOK002 who have received one invitation. Send a second invite to enable exception coding if no response.

Patients with two invitations sent. These are now eligible for exception coding under SMOK002.

๐Ÿงพ ACTION | SMOK002 Chronic disease needs smoking status

Patients with chronic disease who have not had a smoking status recorded this year.
๐Ÿ“…If smoking statuses are gathered during routine chronic disease reviews throughout the year, this report should only need to be reviewed for action during the final months of the contract year. It is recommended to do this after reviewing the corresponding PCA CHECK report for SMOK002.

๐Ÿงพ ACTION | SMOK004 Current smoker needs offer of support

Patients who have been recorded as a current smoker, and no offer of support has been given in the past 2 years.

๐Ÿงพ PCA CHECK | SMOK002 Chronic disease and previous smoking status PCA

Patients with chronic disease who have not had a smoking status recorded, but they have had a PCA recorded within the last 3 years. Review the PCAs to determine if they could be reapplied. 

๐Ÿงพ PCA CHECK | SMOK004 Current smoker with previous PCA

Current smokers who have had a smoking PCA recorded within the last 3 years. Review the PCAs to determine if they could be reapplied.
