004B Routine QOF work | * Indicator Navigator | Stroke & TIA

Stroke & TIA

These searches support achievement of the following QOF indicators for patients with a history of stroke or TIA:

  • STIA007 – Antiplatelet or anticoagulant therapy

  • STIA014 – BP ≤140/90 mmHg in patients aged under 80

  • STIA015 – BP ≤150/90 mmHg in patients aged 80 or over

These searches help manage invitations, borderline readings, high-risk patients, PCA opportunities, and antiplatelet status confirmation.



🔍 [STIA007] One invitation sent

Patients eligible for STIA007 who have received one invitation for medication review. Send a second invite to enable exception coding if no response.

🔍 [STIA007] Two invitations sent

Patients with two invitations sent and no antiplatelet or anticoagulant recorded. These are now eligible for exception coding under STIA007.

🔍 [STIA014] One invitation sent

Patients aged under 80 eligible for STIA014 (BP ≤140/90) who have had one invitation. Send another to support potential exception reporting. If the patient has had a raised blood pressure recorded this year, there must be at least two invitations after the most recent BP result.

🔍 [STIA014] Two invitations sent

Same patients with two invitations and no BP to target. These are now eligible for exception coding under STIA014.

🔍 [STIA015] One invitation sent

Patients aged 80+ eligible for STIA015 (BP ≤150/90) who have had one invitation. Send a second if needed for exception eligibility. If the patient has had a raised blood pressure recorded this year, there must be at least two invitations after the most recent BP result.

🔍 [STIA015] Two invitations sent

Patients aged 80+ with two invitations and raised BP. These are now eligible for exception coding under STIA015.


🧾 BORDERLINE | STIA014 would have achieved surgery BP target

Patients aged <80 whose latest BP reading was home or ambulatory that would have been within target if taken in surgeryHome/ambulatory thresholds are lower than surgery thresholds, so these patients may achieve CHD015 if a surgery reading is taken.

  • Action: Invite for a surgery reading to secure STIA014.

🧾 BORDERLINE | STIA015 would have achieved surgery BP target

Same as above, for patients aged 80+ under STIA015These patients might meet STIA015 if they are invited in for a surgery-based reading.


🧾 CHECK BORDERLINE | STIA014 BP this year not to target NO previous PCA

Patients aged under 80 with BP not to target this year, but no history of PCAs recorded in the last 3 years. These cases may be borderline or newly raised — worth reviewing or repeating.

🧾 CHECK BORDERLINE | STIA015 BP this year not to target NO previous PCA

Patients aged 80+ with raised BP this year, but no history of PCAs recorded in the last 3 years. These may reflect isolated events or measurement variation — worth rechecking.


🧾 HIGH RISK | STIA014 no BP reading this QOF year but latest was raised

Patients aged <80 with no current BP reading and previous raised BP. Likely to fail STIA014 if not followed up.

🧾 HIGH RISK | STIA015 no BP reading this QOF year but latest was raised

Patients aged 80+ with no BP recorded this year, and last reading was raised. Likely to fail STIA015 without intervention.

🧾 HIGH RISK | STIA no BP reading this QOF year but latest was raised

Catch-all list of stroke/TIA patients with no BP this year and previous high readings. Review to prevent indicator failure.


🧾 PCA CHECK | STIA014 BP this year not to target on 3+ antihypertensives

Patients aged <80 who are not to target despite being on 3+ antihypertensives. Likely appropriate for maximal tolerated therapy PCA under STIA014.

🧾 PCA CHECK | STIA015 BP this year not to target on 3+ antihypertensives

Same as above, but for patients aged 80+ under STIA015.

🧾 PCA CHECK | STIA014 BP this year not to target previous PCA

Patients under 80 not to target this year and had a PCA previously recorded. Review for reapplication under STIA014.

🧾 PCA CHECK | STIA015 BP this year not to target previous PCA

Patients aged 80+ not to BP target, with previous PCA applied. Reassess under STIA015.


🧾 QUICK WIN VALUE | STIA no BP reading this QOF year and latest was normal

Patients with stroke/TIA and no BP this year, but latest reading was within target. Ideal for opportunistic BP check — likely to meet STIA014/015 with minimal effort.


🔍 STIA007 Antiplatelet questionnaire to send out by letter

Patients not on antiplatelet/anticoagulant and may require clarification. Use this version to post a questionnaire and establish medication status.
