This folder contains searches to identify patients at high cardiovascular (CV) risk, with a specific focus on blood pressure control, statin prescribing, and HbA1c levels. It is especially useful for practices aiming to optimise outcomes for CHD, stroke/TIA, diabetes, and hypertension indicators.



🔎 CV | 1–4 blood pressure indicators outstanding

These four searches identify patients with 1, 2, 3, or 4 QOF BP indicators not yet achieved:

  • CHD015/016

  • STIA014/015

  • HYP008/009

  • DM036

Use these to prioritise patients with the greatest number of outstanding BP indicators.
