The Palliative Care search folder is part of 012. Audit ready
The Palliative Care folder contains a set of searches and reports which can be used for register and audit purposes.


Most of the searches are based on the QOF Palliative Care register. There are a range of codes which include a patient on the register, some of which are the Gold Standards Framework codes.
  1. RWA-231 to -234 break down the patients on the register who have a GSF prognostic code. The searches are designed so that if two stages are coded on the same date, the higher (eg: red) is the designated stage.
  2. RWA-235 details all patients who do not have a GSF prognostic code. 
  1. RWA-236 is a lesser known code which provides the ability to remove a patient from the palliative care register.

The reports are all very similar and have been designed with audit in mind so it only looks for codes added up until 'today'. This allows users to use a relative run date in the past and obtain data from a previous date without identifying patients had not yet been referred or operated on had the search been run on the intended date (ie: when the actual date is 01/10/2022 but the relative date is 01/04/2022, any patients added after this date will be ignored).

The reports might be useful to reference during palliative care meetings as they extract a number of data points for quick reference.  
 

The patient details include the first line of the home address (primarily to help identify care home), plus the usual GP.
  1. Date added to Palliative Care register - the earliest date of any palliative care related code.
  2. Latest GSF prognostic stage - the latest code of the four stages (red/yellow/green/blue). The date is not necessarily the date the patient was classified with that stage.
  3. Latest GSF supportive care stage - the latest code
  4. Previous GSF prognostic stage - the colour stage preceding the current stage.
  5. DNACPR - the latest code for a CPR decision. This may be for resuscitation or DNACPR, or simply that a conversation has been had.
  6. EOL plan - end of life care plan type codes from the past year.
  7. ReSPECT - the latest ReSPECT form code.
  8. Preferred place of care - the latest preferred place of care, including conversations.
  9. Preferred place of death - the latest preferred place of death, including conversations.
  10. Healthcare involvement - latest codes relating to end of life healthcare teams. Restricted to the last 5 years.
  11. Carer involvement - latest informal carer code. Restricted to the last 5 years.
  12. Residential status - the latest of a set of codes relating to the patient's living status (wider than the codes used by the PCN DES for care homes)
