022 Meds Management: Warfarin control

The Warfarin control searches are designed to identify patients with out of range INRs.  These patients may be reviewed to see if a DOAC could be considered, or if other action to help control their INR needs to be taken.

INR higher than 8
At least 1 INR result of at least 8.0 in the last 6 months.
INR higher than 5 x2
2 or more INR results of at least 5.0 in the last 6 months.
INR less than 1.5 x2
2 or more INR results less than 1.5 in the last 6 months.
INR Time in therapeutic range
Any patient on warfarin who has a coded TTR value ever. Use the report to sort TTR values by percentage, or to identify those who may need an up to date TTR assessed.

All reports have been built to show the Target INR (if coded), all INR results from the last 6 months, plus the out of range result(s) specific to the report.

INRstar contains a tool for auditing TTR, but this information is not routinely coded back to the clinical record. Periodically transferring the TTR to EMIS may be of use to staff who do not routinely use INRstar, as well as for the purposes of wider INR auditing.

Idea
This folder can be used with a relative run date to audit results over time. As a standard these searches all look back 6 months, so 30th June and 31st December would be appropriate run dates to review an entire year.
