IIF-CAN04q) Invalid 2 week rule code used
Prior to the transition to SNOMED, Practices may have used an EMIS code "2 week rule referral....". Patients appearing in this search have had one of these codes added since 01/04/2023, but not a "Fast track referral code". Consider recoding these referrals (and amending any document template properties) to the valid code
IIF-CAN04x) Cancer referrals missed opportunity
Once a cancer referral has been made, the window for getting a QFIT sample technically closes. The attached report highlights patients who do not have a QFIT in the 21 days prior to their referral. Investigate patients to find any uncoded QFIT results. It may be necessary to chase the lab to get a result returned - ensure it is coded BEFORE the fast track referral.
IIF-HI03d) LD healthcheck partially coded
HI-03 requires a learning disabilities review coding AND documenting the completion or review of an LD action plan AND a record of ethnic origin. This search identifies patients who do not have all three codes OR the health action plan was coded before the review.
