Asthma-COPD Overlap Syndrome (ACOS) Template

Purpose

The ACOS template is designed to support the assessment and annual review of patients with Asthma-COPD Overlap Syndrome (ACOS).  It brings together key elements of both asthma and COPD monitoring into a single structure temple, helping clinicians record relevant information efficiently and consistently. 

What Does it Actually Do

The template provides a comprehensive framework for reviewing patients with features of both asthma and COPD. It includes:
  1. QOF data entry for both asthma and COPD indicators
  2. Diagnostic support, including access to spirometry results 
  3. Condition history, including severity, exacerbation frequency, and triggers
  4. Social history, including smoking status, occupational exposure, and housing factors
  5. Clinical examination, including observations and respiratory findings
  6. Management review, including inhaler technique, medication, self management plans, and vaccination advice
  7. Exacerbation assessment and treatment planning 
  8. Links to relevant templates and resources, such as spirometry, smoking cessation, and British Lung Foundation action plan 
The template also highlights mandatory QOF fields to help ensure activity is recorded correctly for payment and reporting purposes.

 Where to find it   

The template can be found on the PCIT Long Term Condition Launcher.   Alternatively, clinician can retrieve the template by pressing F12 key and search 'ACOS'.  


System Dependencies

The template does not require additional SystmOne modules or permissions.    It is important that users are aware that the template exists.
