Advice and Guidance Template
Overview
This template has been created for the following purposes:
- to be used by your team when making advice and guidance referrals which may not be recorded on the Electronic Referral System (eRS)

The template is based on the specification for the General Practice Requests for Advice and Guidance Enhanced Service, but also allows the user to record the outcomes of A&G requests so the practice can audit the associated workload.
There are two sections on the template:
- Referral and related information
- Outcome including any actions needed
How is the template supposed to work in practice?
The idea with this template is that it can be used by team members when making advice and guidance referrals - as per the Enhanced Service these may be telephone calls or sent using other methods and do not have to go via eRS (Choose & Book).
In the detail below, we will cover each section of the template.
Advice and Guidance
The Advice & Guidance section captures the key details required to record A&G requests in line with the 2025/26 Enhanced Service (ES) requirements.
- Tick “Choose and book advice and guidance request” to trigger the appropriate code for claiming.
- Fill in the question for the specialist, date of symptom onset (if known), specialty, and the name of the referrer.
- If the request wasn't initiated by a GP, ensure a GP has reviewed it and tick the relevant box.
For audit purposes, every A&G interaction should be coded even if not submitted via eRS, as long as it meets the ES criteria.
Outcome
Use the Outcome section to document what happened after the A&G request:
- Indicate who took action, the outcome, and any specific advice received.
- Tick relevant boxes if further referral, blood tests, or GP appointments were advised.
These fields support audit, reporting, and appropriate ES claims for follow-up actions.
