Care Navigation

Purpose:

This template is designed to:

  • Support care navigators and reception staff in signposting and care navigation.

  • Provide team members with information and advice to share with patients regarding their symptoms.

  • Enable consistent recording of care navigation and signposting activity for audit purposes.

  • Offer pre-set text messages and hyperlinks that can be sent to patients.

What does it actually do?

  1. Helps users gather information about a patient’s problem or symptoms through structured questions and prompts.
  2. Enables safe navigation of patients to the "Pharmacy First" service using the embedded Care Navigation Pharmacy First protocol.
  3. Guides users on which symptoms or problems can be managed by different roles. (Note: This is a guide only; roles and responsibilities may vary between practices and areas.)
  4. Allows users to record a variety of signposting options and access quick links for follow-up actions.

What does it look like?

The Care Navigation template can be accessed via the Administration Toolbar or by using the F12 launcher and searching for the template.

This page allows the user to:

  • Record that the patient has consented to receive signposting advice.

  • Select the type of encounter and document the source of the referral.

  • Add additional information by clicking the pencil icon next to each question.

It also includes the Care Navigation Pharmacy First protocol, which supports the user in signposting patients to a Pharmacy First service.

*When using the protocol within the template we are aware it double codes into a patients record. We have made TPP aware of this issue and are hoping they are able to rectify the problem.*

If the patient does not present with any of the symptoms listed in the protocol, the template provides a structured set of questions and prompts to gather further information. A Signposting Guide is also available for further support (Note: This is a guide only; roles and responsibilities may vary between practices and areas.)


Outcome 

The second tab of the template is the Outcome page. In this section, users can:

  • Record the signposting advice or service suggested to the patient.

  • Access a variety of quick links to streamline follow-up actions.


The Outcome page also provides:

  • Quick access to helpful external links that can be shared with patients via SMS.

  • Pre-set messages containing advice and self-care information tailored to the patient’s symptoms or concerns.



 

System Dependencies:
This template depends on the user being aware of its existence and the practice or PCN choosing to use it as a way of standardising the recording of their signposting and care navigation activity.
