QOF Explained

Overview

This article has been created to help you understand GP Contract - QOF within SystmOne, Clinical Reporting and the indicators within QOF. These are based on the QOF Business Rules, which can be found here. In Reporting, QOF Indicators, you will find the folders containing all of the QOF indicators/searches. 

Notes
When referring to the indicators in SystmOne, indicators are shown as individual searches with Clinical Reporting, QOF Indicators. For example 'AST005 - Patients on the asthma register' is classed as one of the clinical indicators.

QOF search information

  1. How Am I Driving?: This screen displays the results as of today i.e. it shows the points that would be used to calculate payments if today's date were the end of the financial year. This means that if you continue doing the same things as you have done over the last 15 months or so, these points are what you should expect to achieve at the end of the financial year.
  2. End of Year: This screen usually displays your practice results as of the end of the current financial year (unless you have specified custom dates using the QOF Dates button - see below). This means that if your practice did not record any clinical data after today, the points displayed are what you would achieve at the end of the financial year.

Indicator details

Indicators are broken down into denominator (patients eligible) and numerator (patients included and have met the indicator).

There are 6 columns next to each search/indicator:
  1. Item: The name of the search/indicator
  2. Patient Count: The number of included patients in that search
  3. %: The % column shows the actual % for the number of patients who meet the criteria (out of the parent population)
  4. Target Range: The target % is the percentage you need to reach to achieve full QOF points
  5. Points: This column is the number of points you have and the number of points available for the specific indicator.
  6. Missing Patients: The number of patients that are missing for the indicator to achieve 100%.

Population included/excluded

  1. By right clicking on an indicator you can 'show achieved patients'. This shows the patients that have already attained the indicator.
  2. By right clicking and selecting 'show missing patients' this shows the patients that still need to be targeted to achieved the indicator. This does not include those that have been exception reported or have had the required three invites for a review.
  3. Once you have selected to show your missing or achieved patients you will be presented with a list of patient details whereby you can open the record by right clicking on the patient name.

Export the search results

  1. Below is a step-by-step guide of how to print or export a list of the outstanding patients to be actioned:
    1. Select the chosen indicator and show your list of missing patients.
    2. Right click on any patient in that list .
    3. Select Table
    4. Choose to either display or save the list as a CSV or RTF (which ever you prefer.
    5. This will generate a file with the missing patients in, you can then sort it by Usual GP (to provide a separate list for each GP) or however is appropriate (in line with your practice procedures), for the patients missing m the indicator to be actioned, so the patients become included or have a Personalised Care Adjustment (PCA) added.
             

QOF hints and tips

QOF Target Patients

If you want to quickly access a list of patients for clinicians to work through to achieve maximum QOF points, you can use the Target Patients option at the top left of the SystmOne QOF screen (highlighted in the screenshot below).


For example, you could find the top 50 patients with more than 3 outstanding indicators. Here's how:
  1. Go to the QOF indicators area.
  2. Select Target Patients.
  3. Choose the options you'd like to report on, in this example as selected below:


Once selected, click refresh and it will show the number of alerts and how much each patient is costing the practice:
