Eligible for Diabetes Prevention Programme (HP267)

Purpose:

This protocol will:
  1. Increase the proportion of patients newly diagnosed with NDH that are referred to the Diabetes Prevention Programme

What does it actually do?

The protocol performs the following functions:
  1. Checks when loading the record if the patient has an HbA1c or fasting glucose in the prediabetic range
  2. Check to see if they have been referred to the DPP previously, or have declined it
  3. Makes sure that none of the excluding criteria apply to the patient
  4. Alerts the user that the patient is eligible for the NDPP

What does it look like?


System Dependencies:
Relies on using the clinical system to code HbA1c results and referrals to the diabetes prevention programme

Enable trigger for: All clinical team members
