Frailty not recorded in last 12m (HP346)

Purpose:
This protocol will:
  1. prompt you when a frailty status has not been entered in the last 12 months
  2. prompt you if Frailty Assessment has not been done in the last 12 months.
  3. present a coding template to add the above codes to the record.

What does it actually do?

The alert will first check if the patient is on the frailty register. It will then identify when the last frailty and frailty assessment was coded. If either was more than 12 months ago, an alert will appear to highlight this. The patients current frailty status and when this data was entered is also displayed in the alert text, so the user can be fully informed. This is particularly helpful if you have a Local Specification that covers frail patients.
