Frailty Status and Assessment
Purpose:
The patient status alerts will:
- help to ensure that frailty awareness is optimised within your practice
What does it actually do?
If a patient is 65 or over, the protocol looks to see if the patient has had a code of mild, moderate or severe frailty entered last. If this is the case, the user is alerted to the patients current frailty status so they can review it or update it by clicking on the alert to be taken to the frailty template. If there is no frailty assessment or frailty score recorded but the patient is on the frailty register the user can click on the alert to be taken to the frailty template to undertake a frailty assessment or input their frailty score.

If the user clicks on the alert it will then take them to our frailty template where the patients information can be updated:
System Dependencies:
This relies on users coding activity accurately within the clinical system.
Your practice team need to know that you are setting this alert live in your system
How to get it
These alerts will automatically be in your system when you join our organisation group.
