Lipid Checking Protocol (HP015)

Overview/Purpose

This Lipid Checking Protocol is a protocol for EMIS Web, which has been designed to guide users through managing cholesterol results and to understand the context of the results for the patient that it is being used for.

Resource type:

Base
Optional
Localised

Product

This protocol is part of the PCIT toolset product

What does it do?

The alert is designed to show users:
  1. Whether the patient has familial hypercholesterolaemia
  2. Whether the patient is at risk of familial hypercholesterolaemia and immediate referral or further assessment should be considered
  3. If the patient has established CVD and should be assessed for secondary prevention
    1. checking if the patient complies with CHOL003 for QOF
    2. and also checking CHOL004 for QOF
  4. If the patient has already had a cardiovascular risk assessment already (QRISK 1,2 or 3)
  5. If there are any reasons that QRISK can't be used to assess the patients risk
  6. Then give information about primary prevention depending on whether the patient is on a statin or not
  7. The option to send a text message or letter to the patient about their QRISK level

Why is it important?

This allows users to fully align their approach with the NICE Guidance NG238 Cardiovascular disease: risk assessment and reduction, including lipid modification

What does it look like?

There are a number of steps within the protocol:

Checking for familial hypercholesterolaemia

Lipid safety check

Any of the NICE identified immediate criteria for referral to lipid clinic will be highlighted:

Further lipid investigation

Where thresholds mean that further investigations or assessment are required this is highlighted to the user:

QOF indicators

If the patient is eligible for CHOL003 or CHOL004 and is not achieving these, this is highlighted to the user with further information and possible actions:

QRISK already assessed

If a QRISK has been done since the last cholesterol or in the last year, this is highlighted to the user:

Outside criteria that can be used for QRISK

If the patient has characteristics that mean QRISK can't be used reliably this is highlighted to the user along with useful information:

Primary Prevention assessment

For those already on statins users can see how they are performing according to guidelines on reduction of non-HDL cholesterol:

For those not on statins, previous medications issued are shown as well as wider information about cardiovascular risk and the option to send a patient decision aid via letter or text to the patient:

The communications look like this:
