Eligible for NHS Health Check (HP046)

Overview/Purpose

    To allow staff to check patients to see if they are eligible for an NHS Health Check

What does it do?

When triggered, the protocol evaluates:
  1. The patient's age
  2. Whether the patient has a pre-existing cardiovascular condition
  3. Whether the patient has been issued a lipid lowering drug recently
  4. A previous NHS Health Check in the last 5 years
Based on these criteria, it guides the user step-by-step to determine if the patient is eligible for an NHS Health Check. For some outcomes the user is able to override the warning in order to continue booking an NHS Health Check.

Why is it important?

NHS Health Checks are offered to eligible patients under an Enhanced Service. The eligibility criteria may not be immediately obvious to staff checking the medical record. The checks take a significant amount of clinical time so it's important to ensure that every appointment is funded by the Enhanced Service. This protocol avoids awkward situations in the consultation room by ensuring that only patients eligible will be booked in.

What does it look like?

See below screenshots which show an examples of the protocol in use:


Patients who are not within the eligible age range will trigger the below warning. There is no override so somebody having their 40th birthday next week (ie: they will be eligible on the appointment date) will trigger this alert. Additional consideration should be made for patients currently aged 79 who will be 80 on the appointment date.


Due to the wide variety of conditions there is no provision for showing which conditions make a patient ineligible.






If a patient has declined in the last 5 years, this does not prevent them from changing their mind at a later date.


NHS Health Check not appropriate might have been recorded by a clinician in the past. If this warning appears users may override, but should also consider asking a clinician to review the circumstances into why the code was added.

  1. Clinical coding for cardiovascular conditions
  2. Coding to record previous NHS Health Checks
Clinicians must ensure that this information is recorded and maintained correctly in the system for the protocol to function effectively.
