Trafford High Risk Cardiovascular review alert (HP224)
- highlight to users where patients are eligible for a high risk cardiovascular review for the Trafford Primary Care Quality Contract
What does it actually do?
- The patients record is interrogated upon loading to see if they fulfil the criteria for needing a high risk cardiovascular review (criteria listed below)
- If they fulfil the criteria an alert is displayed to alert the user to the need for a review
- If the user double clicks the alert, they are taken to a data entry template where they can record the review
- Over 40 years
- Latest CVD risk score > 20%
- Not on the following registers: AF, CHD, CKD, Diabetes, HF, Hypertension, PAD, Stroke/TIA, Palliative care
What does it look like?
When the user double clicks on the alert the template looks as follows:
