Trafford High Risk Cardiovascular review alert (HP224)

Purpose:
This protocol will:
  1. highlight to users where patients are eligible for a high risk cardiovascular review for the Trafford Primary Care Quality Contract

What does it actually do?

The protocol performs the following functions:
  1. The patients record is interrogated upon loading to see if they fulfil the criteria for needing a high risk cardiovascular review (criteria listed below)
  2. If they fulfil the criteria an alert is displayed to alert the user to the need for a review
  3. If the user double clicks the alert, they are taken to a data entry template where they can record the review 
Criteria for high risk cardiovascular review:
  1. Over 40 years
  2. Latest CVD risk score > 20%
  3. Not on the following registers: AF, CHD, CKD, Diabetes, HF, Hypertension, PAD, Stroke/TIA, Palliative care
