Purpose:
This protocol will:
  1. help practices to achieve the requirements of the Enhanced Health in Care Homes specification for the PCN DES 2023/24

What does it actually do?

The protocol performs the following functions:
  1. Highlights to users where a patient is eligible for a Personalised Care and Support Plan

What does it look like?



This is a protocol alert which will show in the Zap box in the bottom right hand corner of your screen.
