Weight or creatinine clearance needed for DOAC (HP250)

Purpose:
This protocol will:
- Ensure that monitoring for DOACs is up to date
What does it actually do?
The protocol performs the following functions:
- Checks to see if a patient is on a DOAC
- If they are it will check if a weight and Creatinine Clearance have been done in the last 12 months
- If either of these is missing it will alert the user and allow them to take appropriate action
