Consultation Opener NDH coding check and NDPP referral signposting (HP268)

Purpose:

This protocol will:
  1. Ensures correct coding of NDH and Diabetes as well as offering of the Diabetes Prevention Programme to appropriate patients.

What does it actually do?

The protocol performs the following functions:
  1. Checks on starting a consultation whether the patient has an HbA1c more than 42
  2. If they do and they don't have Diabetes or NDH, the user is alerted to the fact that they may have one of these conditions
  3. The user is then asked if they wish to consider referring the patient to the Diabetes Prevention Programme

What does it look like?

There are a variety of screen prompts depending on the status of the patient.  These look like this:






This Handy Protocol pairs well with HP267, our alert box prompt for NDPP eligibility. You can request for the pair of these to be installed.
