High BP checker (HP387)
Overview/Purpose
This protocol ensures that users achieve the following QOF indicators:
- HYP008 - Hypertensive, age <80, BP 140/90 mmHg or less
- HYP009 - Hypertensive, age 80+, BP 150/90 mmHg or less
- CHD015 - CHD, age <80, with BP 140/90 mmHg or less
- CHD016 - CHD, age 80+, with BP 150/90 mmHg or less
- STIA014 - Stroke, age <80, BP 140/90 mmHg or less
- STIA015 - Stroke, age 80+, BP 150/90 mmHg or less
- DM036 - Diabetes, 79 and under, not frail, with BP 140/90 mmHg or less
What does it do?
- Checks each blood pressure entered to see if the patient is not achieving one of the QOF blood pressure targets
- If they are not, the user is prompted to:
- Consider not entering the blood pressure if the patient has had a normal blood pressure earlier in the year (white coat hypertension/home readings etc)
- Arrange appropriate follow up for the raised reading
- Record maximal tolerated therapy
- Apply a personalised care adjustment for the relevant diseases
- Ask a clinician (via task) for advice
Why is it important?
The QOF targets for blood pressure and cholesterol have a much higher value for the QOF year. Blood pressures are often added where:
- the patient has a home or ambulatory reading which is normal in the contract year
- the patient needs follow up
- the patient is on maximal tolerated therapy
- the patient needs a PCA applying
- the user needs to ask a clinician for advice on further management
Without the PCIT tool, the user would often miss the previously normal reading, or not flag further action required as above. This protocol ensures appropriate action is taken to achieve the QOF target.
