We have structured our template into 5 sections:
KIT Management: Record any exceptions or note that you have given the patient the FIT kit and leaflet, including the date.
Planned Actions: Record what you plan to do once the results are returned.
Test: Record that the sample has been sent to the lab, including the date.
Results: We have embedded a view of the qFIT results along with other pathology results, past consultations, and follow-up notes to ensure you follow through on your planned actions.
Actions Following Results: Record the actions taken, such as a fast-track referral, a re-test of the FIT, or follow-up appointments.
