CAN 04: FIT Template

Purpose:
This template is designed to help users record information to fulfill cancer referral requirement.    This includes information about FIT kit provision, planned action, Test request and the Actions Following Results.    The template is linked to the Patient Status Alert that is displayed in the patient's records. 

What does it look like?

Template      

We have structured our template into  5 sections:
  • KIT Management: Record any exceptions or note that you have given the patient the FIT kit and leaflet, including the date.     
  • Planned Actions: Record what you plan to do once the results are returned.
  • Test: Record that the sample has been sent to the lab, including the date.
  • Results: We have embedded a view of the qFIT results along with other pathology results, past consultations, and follow-up notes to ensure you follow through on your planned actions.
  • Actions Following Results: Record the actions taken, such as a fast-track referral, a re-test of the FIT, or follow-up appointments.  

  • If the user chooses to add one of the reasons a FIT is not indicated they are provided with this option:

    System Dependencies:

    The use of this alert is helped by the user making use of the PCIT Suspected Colorectal Cancer templates to record activity leading up to and after the referral.  This template is connected to two patient status alerts

    Fitting your practice

    Your team need to discuss and be clear about how you are consistently recording the provision of FIT tests within the clinical system, so this alert and the supporting information helps to support you.
