Percentage of patients on the QOF Learning Disability register aged 14 or over, who received an annual Learning Disability Health Check and have a completed Health Action Plan

This indicator should sit over the existing Enhanced Service. When the annual review is done, ensure a health action plan is created or reviewed on the same date or after.
Any health action plan coded before an annual review will not be counted.
