Hormone injections and implants are used in oncology, gynaecology, and other long-term conditions. They require specific safety checks and patient counselling. This template ensures:
  • Safe prescribing and administration โ€“ with pregnancy checks and anti-androgen cover where appropriate

  • Oncology monitoring โ€“ including PSA or treatment plans

  • Patient counselling โ€“ about side effects, bone health, and contraception

  • Accurate coding โ€“ for audit and continuity of care

The template follows the same workflow as the Generic Injection Template:
  1. Pre-procedure checks โ€“ risks, consent, safety checks

  2. Administration details โ€“ type of hormone therapy, route, site, technique

  3. Post-procedure documentation โ€“ outcome, advice, sharps disposal, Yellow Card if required

It also includes hormone-specific sections:

  • Indication (prostate cancer, endometriosis, breast cancer, other)

  • Oncology monitoring plan (PSA or specialist plan)

  • Pregnancy status checked (not pregnant, excluded by test, or not applicable)

  • Side-effect counselling (hot flushes, mood changes, injection site reactions, reduced bone density, sexual dysfunction, contraception advice)

  • Anti-androgen cover (already given, not indicated, declined, or not applicable)

  • Bone health plan (DEXA scan, vitamin D/calcium advice, under specialist care)

  • Course status (started, continued, completed, stopped)

๐Ÿงพ Local Enhanced Service (LES) Section

  • Where a Local Enhanced Service exists, a section will automatically appear at the top of the template.

  • This ensures correct coding and claim submission.

  • No separate LES template is needed.

๐Ÿ› ๏ธ Pre-procedure

  • Risks discussed (pain, bleeding, infection, allergic reaction, fainting)

  • Consent obtained and recorded

  • Injection under PGD or PSD

  • 6 safety checks completed

  • Vaccine cold chain confirmed (if relevant)

๐Ÿ’‰ Administration

  • Record if injection or implant given: SC, IM, or implant insertion

  • Site: abdomen (left/right) or other

  • Technique helpers: aseptic technique, Z-track/skin tent, local anaesthetic

  • Confirm injection/implant delivered using recommended technique

โœ… Post-procedure

  • Outcome: no reaction, faintness, pain/swelling, allergic reaction

  • Sharps disposed of safely

  • Aftercare advice documented

  • Yellow Card submitted if adverse reaction suspected
