Last Smear Declined (HP180)

Overview

HP180 is a simple protocol that will appear if the patient has declined a smear and is eligible for one. The alert will disappear if the patient has had a smear after they declined. This is useful to enable the member of staff who may be talking to this patient to encourage them to attend, or if the patient reports any cervical health concerns.

The alert will not appear for:
  1. Patients with a "male" flag
  2. Patients younger than 25 or older than 65
  3. Patients who have a hysterectomy
  4. Patients recorded as pregnant in the last 9 months
  5. Patients who have had a smear, or a user has added a code to say a smear should not be undertaken, in the last 3.5 or 5.5 years.

It is important to understand that unless you have a different local process in place, practices should not be inviting patients for smears based on EMIS searches; 1st and 2nd invites are issued via the national screening programme and this information can be seen in the Cervical Screening Management System (CSMS), practices are then responsible for sending out a 3rd invite to non-responders using the CSMS "non-responder list".

What does it do?

When triggered, the protocol evaluates:
  1. Which age timeframe the patient falls into - 25 to 52 or 53 to 64
  2. The timeframe for declines is then checked: 3.5 years for the younger cohort and 5.5 years for the older cohort.
  3. An alert will then appear informing the user that the patient has declined their latest smear, the date they declined and will also show the latest smear if there ever has been one
