This shows a step by step workflow for sending Fit Notes to patients using Accurx ChainSMS service with the new functionality to attach documents.

 Initially create your sick note by following steps:
 
 - In consultation mode click on "Add fit note"
 
 - Decide on new issue - not fit or may be fit, or duplicate/reprint fit note

 
 
  
   
    HINT: If you create a problem, this will automatically pull through as the reason for the fit note when creating it.  It will also helpfully link the fit note to this particular problem for future information.         
   
  
 
  Go through and complete the fit note information then:
  
 
 
  - In printer settings click on "Printer Name" and select "Microsoft Print to PDF"
 
  - Click on "Preview" 
 
 
 
  
   
    
     DO NOT click on "Print" as this will crash EMIS                                                                                                                                                                                                                                                                   
     
    
   
  
   The following screen will appear, click on "Print":
   
  
  
  
 
  
 
 
  The following warning will appear, click "Yes":
  
 
 
 
  
 
 
  In the "Save Print Output As" box that then opens, select where you want to save the form - we usually use Desktop (1), add a file name (2)  and then click save (3):
  
 
 
 
  
 
 
  Now, the system will return you to EMIS where you need to save the patient record:

  Open the file you've just saved (from the location you saved it in) in Adobe Acrobat:
 
 
 
 
 Click on "View" and "Rotate view" selecting Counterclockwise:
 
 The fit note should now show in the correct orientation as below.  Click on "Fill & Sign" on the right hand bar:
 
 
 The following box will ask who needs to fill and sign - select "You" and click on "Fill and sign"
 
 This will return you to the fit not with the box "sign" available at the top.  Click on this:
 
 Select "Add Initials"
 
 You can then 
 
 - Select "Draw"
 
 - Draw your signature
 
 - Select "save initials"
 
 - Click "Apply"
 
 
 You can then drag and drop this signature to the appropriate place on the form:
 
 
 
 Then on the top bar select "Next":
 
 You will see the following Prompt box:
 
 Save the file to overwrite your original file (same location and same file name) with the next box and click "Yes" to the overwrite prompt

  Open a text message to send to the patient.
  
 
 
  Type in a message to the patient and then click "Attach file"
  
 
 
 
 
 You should then see the attached file
 
 Click "Send and save" at the bottom in blue and you're done!
