Using the High Risk Drug Monitoring Templates
Purpose
High-risk drug monitoring templates have been designed to support safe and consistent monitoring for patients prescribed medicines that require regular review and blood test surveillance.
These templates help clinicians ensure that essential checks are completed and documented according to current clinical guidance — reducing risk, improving patient safety, and providing a clear audit trail within SystmOne.
There are currently two main groups of high-risk drug monitoring templates available:

DMARD (Disease-Modifying Anti-Rheumatic Drugs) – e.g. methotrexate, sulfasalazine, hydroxychloroquine.

Non-DMARD High-Risk Drugs – e.g. amiodarone, lithium, warfarin and other medicines requiring close biochemical or clinical monitoring.
What does it actually do
Each template includes:
Pre-populated fields for relevant baseline and ongoing tests (e.g. FBC, LFT, U&E, TFT, INR).
Clinical safety prompts and review reminders.
Space to record shared-care arrangements, specialist correspondence, and advice provided to patients.
When opened, each HRDM template automatically:

Pulls in relevant latest pathology records where available (so you can review trends at a glance) or manually records results that have not been integrated into the patient's records i.e. hospital blood results.

Displays key BNF monitoring guidance for that specific medicine.
Provide views of important patient information such as allergies and sensitivities, medication

Provides structured fields for each monitoring parameter (e.g. LFT, FBC, TFT, U&E).

Allows tick-box confirmation for essential safety steps (e.g. ECG done, hospital results checked, red flag advice given).

Links to related templates such as the medication review template for a full structured medication review, pathology requests, letter template, or patient information leaflets — all from the same screen.

Creates an auditable record within the patient’s notes to show monitoring has been completed safely and according to protocol.

Supports follow-up work flow e.g. scheduling tasks, recalls, or blood test reminders directly from the template.

Users can access the above template either from the Medicine Management template (this icon
) which is on the side toolbar on all of the PCIT templates. We have also created a link to the Medicine Management template on our OneTemplate for clinicians and Non-Prescriber OneTemplate.

System Dependencies
This template depends on the user being aware of its existence and the practice or PCN choosing to use it as a way of standardising the recording of their structured medication review. Users will also benefit from other SMR resources that have developed.
