To complete a valid NHS Health Check, you must:

  • Record:

📮 Invite and Admin

  • Tick to indicate invitation sent (SMS, letter, phone)

  • Use this to populate invite/uptake audits

🚬 Smoking

  • Status (never, ex, current)

  • Option to record smoking cessation advice

  • Note: e-cigarette use alone does not count for QOF smoking indicators

🍷 Alcohol

  • Document if the patient is an ex/non-drinker or current drinker

  • Record units/day and week

  • AUDIT-C screening (3 questions)

  • If score ≥5 or weekly alcohol >14 units: complete full AUDIT questionnaire

  • AUDIT-C questions are mandatory for drinkers — continue to full AUDIT if:
