This template supports delivery of the NHS Health Check Programme, a nationally commissioned service for adults aged 40โ€“74 without pre-existing cardiovascular disease.

To complete a valid NHS Health Check, you must:

  • Record:

    • Demographics and lifestyle data

    • Alcohol and smoking status

    • BMI, blood pressure, and cholesterol

  • Calculate:

    • QRISK3 score

  • Provide:

    • Brief lifestyle advice

    • Referrals if appropriate

  • Tick to indicate invitation sent (SMS, letter, phone)

  • Use this to populate invite/uptake audits

๐Ÿšฌ Smoking

  • Status (never, ex, current)

  • Option to record smoking cessation advice

  • Note: e-cigarette use alone does not count for QOF smoking indicators

๐Ÿท Alcohol

  • Document if the patient is an ex/non-drinker or current drinker

  • Record units/day and week

  • AUDIT-C screening (3 questions)

  • If score โ‰ฅ5 or weekly alcohol >14 units: complete full AUDIT questionnaire

  • AUDIT-C questions are mandatory for drinkers โ€” continue to full AUDIT if:

    • Score is 5 or more

    • Units exceed 14/week

  • Ensure QOF-compliant smoking coding even if e-cigarette use is noted

  • Can be used in stages (e.g. HCA does initial data, clinician completes)
