This template supports claims for blood samples taken under a Local Enhanced Service (LES). To be valid for payment:
The "Blood sample taken" box must be ticked
Documentation must clearly show that a sample was successfully collected
Correct completion ensures:
Accurate funding claims
Audit-ready records
A clear process for consent and communication
Many LES agreements reimburse practices for blood tests taken in-house, especially where they support secondary care or long-term condition monitoring. This includes:
GP-requested bloods
Shared care protocols (e.g. methotrexate, lithium)
Glucose tolerance testing (GTT)
Community and hospital referrals
This template includes:
Tick Consent given for blood test
Tick Blood sample taken (required for payment)
Leave unticked if the patient did not attend or blood was not taken
Free-text field to specify details if this was a GTT:
Baseline vs. post-load values
Timing of test
Reason (e.g. gestational diabetes screening)
Tick Yes if the bloods are in support of a hospital or specialist request
This supports LES claims where primary care carries out investigations on behalf of secondary care
