Phlebotomy

๐Ÿ’‰ Phlebotomy

๐Ÿ’ท How much is this worth to me?

This template supports claims for blood samples taken under a Local Enhanced Service (LES). To be valid for payment:

  • The "Blood sample taken" box must be ticked

  • Documentation must clearly show that a sample was successfully collected

Correct completion ensures:

  • Accurate funding claims

  • Audit-ready records

  • A clear process for consent and communication

๐Ÿ“Š High-level overview of the specification requirements

Many LES agreements reimburse practices for blood tests taken in-house, especially where they support secondary care or long-term condition monitoring. This includes:

  • GP-requested bloods

  • Shared care protocols (e.g. methotrexate, lithium)

  • Glucose tolerance testing (GTT)

  • Community and hospital referrals

๐Ÿ“Œ What do you need to know?

This template includes:

โœ… Consent and Sample Collection

  • Tick Consent given for blood test

  • Tick Blood sample taken (required for payment)

  • Leave unticked if the patient did not attend or blood was not taken

๐Ÿงช Glucose Tolerance Test (GTT)

  • Free-text field to specify details if this was a GTT:

    • Baseline vs. post-load values

    • Timing of test

    • Reason (e.g. gestational diabetes screening)

๐Ÿฅ Secondary Care Request

  • Tick Yes if the bloods are in support of a hospital or specialist request

  • This supports LES claims where primary care carries out investigations on behalf of secondary care

๐Ÿ“‹ Referral Pathway

Record where the patient is being referred for phlebotomy:

  • Practice-based phlebotomist

  • Community service

  • Hospital-based phlebotomist

This section also supports workload coding and claim separation.

๐Ÿ“ฑ Communication Consent

  • Tick if the patient agrees to receive test results via SMS

  • Enables compliant communication aligned with practice data protection policy

โŒ Did Not Attend (DNA)

  • Tick if the patient did not attend their phlebotomy appointment

  • Use to support failed encounter documentation and rescheduling workflow

๐Ÿ’ก Hints and Tips

  • Only tick "Blood sample taken" if the sample was actually drawn โ€” this drives payment

  • Use OneDiary or follow-up templates alongside this for tracking next test due dates

  • For shared care drugs, link this template with structured monitoring templates (e.g. methotrexate)

  • Consider batch entry if multiple patients are seen in a phlebotomy session for audit purposes
