Steps for Recording a Patient Review in EMIS

To ensure that a patient review is correctly documented:

1. Accessing the Patient Record

  1. Open EMIS Web and search for the patient’s record.

  2. Navigate to the OneRecall alert at the bottom right of the patient’s screen.

  3. Double-click the alert to open the Recall Review Template.

2. Completing the Review

Within the recall template, complete the following sections:

  • Review Outcome:

    • Tick Chronic Disease Annual Review Completed if the recall is now complete.

Utilise OneMonitoring if you want more information about what is needed for QOF or address data entry as required by double clicking on it.

3. Setting a Follow-Up Recall

If additional monitoring is required before the next scheduled review:

  1. Tick Recall Sooner Than Month of Birth (MOB).

  2. Select the follow-up interval (e.g., +6 months, +1 year).

  3. Save the record to schedule an interim recall.

Handling Non-Engaged Patients and Exception Reporting

For patients who do not attend or decline follow-ups, use OneMonitoring to add the appropriate information

  • Standardise Entries: Ensure all clinicians follow the same documentation format for consistency.
