Self-Management Plan (AST007)

✅ Self-Management Plan (AST007)

This shared component enables clinicians to meet the requirements for QOF indicator AST007 by documenting or updating a written personalised asthma action plan. It also supports safer patient self-management and improved clinical outcomes.

🧭 When Will I See This?

  • During a full asthma review (especially where QOF indicators are outstanding)

  • When building or updating a written asthma action plan

  • When completing the review closure section of the template

📐 Peak Flow Zone Calculation

  • Enter best-ever PEFR or use the calculator to estimate predicted PEFR

  • System will calculate:

    • ✅ Green Zone (80–100%)

    • ⚠️ Amber Zone (50–80%)

    • ❌ Red Zone (<50%)

These can be used in the written action plan or to inform safety-netting advice.

📈 Why This Matters

  • Required for QOF AST007 (annual written plan)

  • Improves patient confidence and early recognition of flare-ups

  • Supports safety-netting, especially for high-risk or poorly controlled patients

  • Reinforces inhaler usage and emergency instructions

🧠 Ensure the plan is saved and ideally printed or emailed to the patient for their records.
