💷 How much is this worth to me?

This template directly supports achievement of QOF asthma indicators and smoking indicators, helping ensure full payment for patients on the asthma register. Specifically, it supports:

  • AST007 – Annual asthma review including written self-management plan

  • SMOK002 – Current smoking status recorded

  • SMOK004 – Smoking cessation advice offered to smokers

Incorrect coding or missing entries can result in indicator failure, even if the care has been delivered.


📊 High-level overview of the specification requirements

AST007: Asthma Review

You must record an annual asthma review that includes:

  • Discussion of symptom control

  • Provision of a written self-management plan

  • Review of exacerbations and medication

SMOK002: Smoking Status

All patients aged ≥15 must have a smoking status recorded within the last 15 months.

SMOK004: Smoking Cessation

All current smokers must be offered smoking cessation advice or support annually.

Patients can be removed from these indicators via personalised care adjustments (PCAs), including:

  • Declining reviews or support

  • Maximum tolerated therapy

  • Exception reporting via appropriate invitation recording

This template includes all required elements for contract achievement:

🔍 Asthma Review Section (AST007)

  • Record number of exacerbations in the past 12 months

  • Tickboxes to confirm:

    • Self-management plan provided

    • Asthma review undertaken

🚬 Smoking Status and Advice (SMOK002 & SMOK004)

  • Select current smoking status (including declined)

  • Record smoking cessation advice if appropriate

  • Tickboxes for declined support

⚙️ Personalised Care Adjustments (PCAs)

Options to:

  • Remove patient from all asthma indicators

  • Remove patient from all smoking indicators

  • Record invitations (e.g. 2+ invites 7 days apart for exemption)


💡 Hints and Tips

  • Tick both boxes for AST007: one for the review, one for the self-management plan — both are required.

  • If the patient declines smoking status or cessation support, make sure to code this explicitly to enable PCA.

  • Use the bottom section of the template to record invitations and document exemption attempts.

  • Where local enhanced asthma reviews exist, these will also appear in the template to avoid duplication.

  • Double-check that the review date is within the current QOF year — a common reason for indicator failure.
