This template supports performance against key QOF indicators for patients with COPD:
COPD015 – Annual review including MRC Dyspnoea score and exacerbation count
SMOK002 – Smoking status recorded
SMOK004 – Smoking cessation advice offered
Correct use ensures patients are accurately reviewed, appropriate lifestyle interventions are documented, and income is not lost due to incomplete coding.
You must complete and code:
MRC Dyspnoea Score
Exacerbation count in the past 12 months
Annual COPD review
Reviews can also be documented at 3 or 6 months if relevant, but only the annual review counts for QOF.
Smoking status must be recorded for all patients aged 15 or over.
Current smokers must be offered smoking cessation support or advice annually.
This template includes the following components:
Choose one of five grades:
Breathless only on strenuous exercise
Short of breath when hurrying or walking uphill
Walks slower than others, stops after a mile or 15 minutes
Stops after 100 yards or a few minutes on the flat
Too breathless to leave the house or dress

Required for exception coding (e.g. 2 invites 7+ days apart)
COPD015 requires three things: MRC score, exacerbation count, and annual review coding.
Smoking indicators apply to all COPD patients aged 15+ — always record even if unrelated to review.
Use appropriate PCA or invitation coding for non-attenders or patients who decline monitoring.
Smoking cessation advice must be recorded each year even if already declined previously.
Consider using structured diary dates or recall systems to avoid missed annual reviews.
