This template supports QOF delivery for two core hypertension indicators:

  • HYP008 – Blood pressure control in patients <80

  • HYP009 – Blood pressure control in patients β‰₯80
    It also captures required data for:

  • SMOK002 – Smoking status

  • SMOK004 – Smoking cessation advice

HYP008: BP Control <80 years

  • Target: ≀140/90 mmHg

  • If using home/ambulatory BP: ≀135/85 mmHg

HYP009: BP Control β‰₯80 years

  • Target: ≀150/90 mmHg

  • If using home/ambulatory BP: ≀145/85 mmHg

SMOK002 & SMOK004

  • All patients 15+ should have smoking status recorded (SMOK002)

  • All smokers should be offered cessation advice yearly (SMOK004)

This template captures the following:

πŸ”’ Blood Pressure Recording (HYP008/009)

Fields to enter:

  • Latest clinic systolic/diastolic

  • Average home BP (systolic and diastolic)

  • Ambulatory BP (systolic and diastolic)

If target not met:

  • You must either intensify treatment or

  • Record a PCA (e.g. max tolerated treatment)

❌ BP Exception Reporting

  • Tick if BP can’t be taken or controlled

  • Acceptable reasons:

    • Maximum tolerated therapy

    • Refusal

    • Other clinical unsuitability

🚬 Smoking Status (SMOK002)

  • Dropdown to record current smoking status

  • Option to tick "declined"

🚭 Smoking Cessation Advice (SMOK004)

  • Tick to confirm advice given

  • Tick "declined" if patient refused advice


πŸ’‘ Hints and Tips

  • Use home/ambulatory readings where in-clinic BP remains above target – thresholds are lower but often easier to meet.

  • Ensure systolic and diastolic values are entered β€” incomplete readings can result in indicator failure.

  • Apply PCA codes annually if patient remains uncontrolled or if treatment is clinically inappropriate.

  • Review all patients with elevated BP prior to QOF year-end to ensure values are updated.

  • Even if BP is well controlled, smoking status and advice must still be recorded for patients β‰₯15.
