This template supports achievement of the following QOF indicator:
NDH002 โ Recording an annual HbA1c result for patients on the NDH register
Meeting this indicator ensures continued funding under the QOF framework and supports early identification of patients at risk of developing Type 2 diabetes. It may also contribute to enhanced service contracts in some areas.
Patients on the NDH register must have an HbA1c recorded each QOF year
If not possible, a valid Personalised Care Adjustment (PCA) must be applied:
Declined testing
Clinically unsuitable
Patient has opted out of NDH monitoring
This template includes the following fields:
Record the latest HbA1c value in mmol/mol
This value will be used to confirm whether the indicator has been met for the year
Options include:
Blood glucose test declined
Patient is unsuitable for NDH indicators
Patient has declined NDH indicators
Recording one of these will exception code the patient from the NDH002 indicator for the QOF year.
If the patient has not attended, you can record up to two invitations (7+ days apart) to qualify for exception reporting
These should be coded appropriately using the dropdown menu provided
Ensure HbA1c is recorded within the QOF year โ values from the previous year will not count.
The same blood result may also satisfy diabetes screening or enhanced service requirements if appropriately coded.
Use PCA codes only when clinically justified or when the patient declines testing after invitation.
Where testing is overdue, consider linking this template to recall or diary date processes to flag patients due review.
Remember that NDH codes are used to track patients at risk โ timely intervention here helps prevent diabetes development.
