This template supports achievement of the following QOF indicators:
STIA014 / STIA015 β Blood pressure control in stroke/TIA patients
AF007 (in stroke/TIA context) β Use of an antiplatelet or anticoagulant
SMOK002 / SMOK004 β Smoking status and cessation advice
Accurate completion ensures correct QOF reporting and supports local audits of stroke secondary prevention.
Under 80 years: Target β€140/90 (or β€135/85 if home/ambulatory)
80+ years: Target β€150/90 (or β€145/85 if home/ambulatory)
Patients with stroke/TIA should be prescribed:
Aspirin or
Clopidogrel or
Anticoagulant
Declines or contraindications must be coded for each option not used
SMOK002 β Record current smoking status for all patients aged β₯15
SMOK004 β Offer annual cessation support to all current smokers

Tick exception box if BP canβt be recorded or controlled
Tick whether the patient is on:
Aspirin
Clopidogrel
Anticoagulant
Record declined or contraindicated for each type if none used
A valid reason must be provided for each if target is not met
Drop-down for current smoking status
Tick if patient declined to provide it
Record whether cessation support was given or declined

Use for permanent exclusions (e.g. frailty, clinical unsuitability)

If the patient is not taking any antiplatelet or anticoagulant, all three exemption reasons must be completed to achieve AF007 by exception.
Home or ambulatory BP readings often help meet targets when clinic readings are high.
Smoking indicators apply to all patients aged 15+, not just smokers.
For patients with multiple stroke-related conditions (e.g. AF + TIA), ensure anticoagulant vs. antiplatelet choices are clearly documented.
Always check whether a patient is already coded as declined or contraindicated from previous years β reuse where valid.
