Medicine Management/Structured Medication Review
Purpose
Our Medicine Management template will ensure optimal performance for the Structured Medication Reviews specification of the PCN DES. It is designed to provide a systematic and efficient approach to reviewing a patient’s medications. This template depends on the user being aware of its existence and the practice or PCN choosing to use it as a way of standardising the recording of their structured medication review. Users will also benefit from other SMR resources that have developed. Please also see 1) Target for SMR protocol
