What does it look like?

If you want to review the template in full, please attend one of our regular training sessions for Admin team members or speak to your customer relationship manager.  Some examples of the template pages are shown below:

Important information page

This page introduces the template and provides visual colour guidance for navigating different elements. It includes direct links to the support team, enabling quick troubleshooting or the submission of development requests. Users can also find the office contact number, and automatically-generated details confirm when the template was used.
This section pulls together key information to identify records such as the patient's preferred method of contact, any visual or hearing impairments, and any interpreter needs, which can be found in the Communication needs section.  Housebound, care home status and any carer details can also be found on this page.

Appointments

In this page, any opportunities for QOF reviews are shown for the patient. Each condition will be it's own section in this page, and will include a drop down box to select the type of invite, or if the review has been declined, or deemed unsuitable.
There are more sections than show in the example screenshot (Cancer, COPD, Diabetes, Mental Health, Heart failure just to name a few) as these will only appear for patients who are diagnosed with the appropriate condition.

Prescription admin

This page can be used to record prescription collections, urgent requests, and if a prescription has not been collected.

System Dependencies:

There are no system dependencies.   As with all Primary Care IT templates, these are highly dynamic and rely on appropriate coding of problems within patient records.
