Purpose
Primary Care IT has developed a various set of patient questionnaires within SystmOne. These questionnaires allow practices to collect important clinical information directly from patients in a structured and standardised way. Patients can complete reviews, request services, and provide health data remotely, which helps reduce administrative burden and improves patient engagement. Once summitted and reviewed by a clinicians, the results are then filed into the patient records.
What do they do
The questionnaires allow patients to submit health information securely to their GP practice without needing a face-to-face appointment. Information is received directly into the patient record, supporting clinicians in reviewing long-term conditions, processing medication reviews, sending advice information or managing fit note requests. This makes care more efficient and ensures practices are capturing standardised data for clinical reviews and contractual requirements.
How to use them
Users can use the questionnaires either by sending them out through SMS, email or Airmid notification to the patients depending on the Practice's local preference and settings.
Click on to the 'Communication Annex' icon on the toolbar (require admin configuration).
The same icon can also be found within the side toolbar appearing on all PCIT templates as shown below
2. Under the Message section, choose method of sending (i.e. SMS, email or Send Airmid notification) depending on patient preference.
3. Under the Questionnaire section, click on to the
icon which will bring up all active PCIT questionnaires. Choose the required questionnaire from the available list.
4. Enter the message text to the patient or use the preset if already set up in the main box and click 'send'
5. When a patient completes the questionnaire and submit the form, it will return to the patient’s record (via SystmOne task - unassigned) for review by the clinician

System Dependencies
There is no system dependencies for SystmOne Questionnaire. However, the Practice must have a clear internal process as outlined below
- Communication configuration: SMS, Email or a chosen patient communication method must be configured correctly
- Staff responsibilities: All relevant should understand how returned, how to process the incoming task, and when to notify the clinician to review the completed questionnaire.
- Follow up actions: If further clinical action is required, this should be clearly documented and escalated according to the Practice's internal protocol.
