Using the QOF Template

Introduction

This support article is designed to help clinicians understand and effectively use the Primary Care IT QOF template within SystmOne. The template has been developed to streamline QOF activity recording by bringing together key indicators and their associated actions into a single, accessible interface.

By guiding clinicians directly to the relevant indicator sections, the template helps ensure that required QOF activity is completed and coded correctly during the consultation.

1. Template overview

The QOF template cover page is divided into two main sections designed to guide the clinician quickly to the required work.



A. Outstanding QOF Alerts (Top Section)

The top section displays a list of outstanding QOF alerts for the patient currently open in the record.
This area acts as a quick visual summary showing:
  1.       Which QOF indicators still require action
  2.       What clinical activity is outstanding
  3.       Which indicator the action relates to
    For example: Blood pressure - BP002: Record blood pressure. This allows clinicians to immediately identify what needs completing without navigating through multiple templates. 

B. Indicator Navigation Panel (Botom Section)

The bottom section provides direct navigation buttons to each QOF clinical areas, such as: AF, Astham, CHD, CHOL, COPD, Dementia, Diabetes, HF, HYP, NDH, Stroke. Selecting on of these buttons will take the user straight to the corresponding QOF indicator template for that condition.

C. Patient -Relevant Indicator Only

To improve efficiency and prevent unnecessary navigation, the template is designed so that the only indicators relevant to the current patient can be launched. If a clinician select an indicator that is no relevant to the patient, the system will display a message prompting the user that the indicator is not applicable. See below


D. Removing Patients from QOF Indicators


If patient needs to be removed from a specific QOF indicator (for example, through exception code), clinicians should use the QOF exception Codes link within the corresponding indicator template. This ensures 1) The correct exception coding is applied 2) The patient is appropriately excluded from the indicator 3) QOF reporting remains accurate. Below is an example of a link to exception on the Asthma QOF template


E. Additional Template Navigation

Each page of the QOF template contains a shortcut link located at the top right of the side toolbar which provide access to the full assessment page for that indicator. Selecting this button will direct the user to the specialist page for the currently selected QOF topic.



For example, if the user is currently viewing the Asthma Page, selecting this button will open the Asthma specialist page.

  2. Quickly access the full specialist template or detailed assessment page
  3. Review condition-specific information
  4. Complete additional clinical activity that may sit outside the core QOF indicator
This ensures clinicians can move seamlessly between QOF indicator recording and more detailed review template without needing to manually search for the relevant template.

2. How to find the template

1. If configured at the practice level, the QOF template icon will be visible on the SystmOne toolbar.
