Using the QOF Template
This template allows you to record everything you need for QOF. It covers Asthma, Atrial Fibrillation, Cancer, Coronary Heart Disease, COPD, Dementia, Depression, Diabetes, Heart Failure, Hypertension, Mental Health, Rheumatoid Arthritis and Stroke.

 

To use the template:
1. Click on the QOF button on the PCIT Toolbar


2. From the Cover Page on the required page

3. Each page of the template has the usual useful right hand side link – the QOF template has an extra button at the top of this list which will take you to the specialist page for that particular topic.

4. Complete the relevant fields within the template page.


Outstanding items

The bottom of the QOF Template displays outstanding QOF items – these will be displayed if a patient has not had a relevant QOF action. Ie If a patient is in the Report: AF007 - Missing patients (work to do) the AF007 would be active


 Click on Launch Communications Annex

 Click on the Questionnaire button

 

 Search for the required Questionnaire – ie AF007

Click OK

Type an appropriate message.

Click Send
