This folder contains searches to identify patients at high cardiovascular (CV) risk, with a specific focus on blood pressure control, statin prescribing, and HbA1c levels. It is especially useful for practices aiming to optimise outcomes for CHD, stroke/TIA, diabetes, and hypertension indicators.
These four searches identify patients with 1, 2, 3, or 4 QOF BP indicators not yet met:

Use these to prioritise patients with the greatest number of outstanding BP indicators.
Identifies patients recently escalated on treatment but still not at BP target.
These patients may need follow-up blood pressure readings to confirm improvement.
Patients with moderately raised BP and no PCA recorded within the last 3 years, or new medication added in the last 6 weeks.
Patients with severely raised BP and no recent action taken.
Consider urgent review or escalation.
Patients with mildly raised BP that may still affect QOF achievement if left unaddressed.
Patients with LDL >2.0 mmol/L this year who may benefit from treatment intensification.
Patients just above the threshold LDL level (i.e. between 2.0 and 3.0), especially relevant for patients with CVD or diabetes.
If no LDL level present, similar nHDL range 2.6 to 3.6 is shown.
Supports DM020
Non-frail patients with raised HbA1c results requiring therapy review or intensification.
Supports DM021
Frail patients with significantly raised HbA1c.
Consider review or PCA depending on clinical context.
Patients not on statin therapy, eligible for QOF indicators, with no PCA coded.
Useful for identifying patients who may need therapy initiated.
