Indicator ID
Description
CAN004
The percentage of patients with cancer, diagnosed within the preceding 24 months, who have a patient Cancer Care Review using a structured template recorded as occurring within 12 months of diagnosis.

What do the business rules mean?

What makes a patient eligible?

Patients are eligible for the CAN004 denominator if:
  1. they have been diagnosed with cancer during the current contract year

    or
  1. they were diagnosed with cancer during the previous contract year, but did not have a Cancer Care Review completed in the 12 months following diagnosis 

How to achieve the indicator

Conduct a Cancer Care Review using a structured template within 12 months following the cancer diagnosis.

What will remove patients from eligibility?

  1. Patients who were diagnosed within the current contract year and they haven't had a cancer care review (they will also appear in next year's denominator)
  2. Patients for whom cancer care quality indicator care was unsuitable in the contract year
  3. Patients who chose not to receive cancer care in the contact year
  4. Patients who have not responded to at least two cancer care review invitations (at least 7 days apart) in the contract year
  5. Patients who were diagnosed with cancer within the last 3 months of the contract year
  6. Patients who were newly registered with the practice within the last 3 months of the contract year
Notes
If a patient achieves the indicator at any point during the year, any exclusions that have also been added are ignored.
