| Description |
CAN004 | The percentage of patients with cancer, diagnosed within the preceding 24 months, who have a patient Cancer Care Review using a structured template recorded as occurring within 12 months of diagnosis.

What do the business rules mean?
What makes a patient eligible?
Patients are eligible for the CAN004 denominator if:
- they have been diagnosed with cancer during the current contract year
or
- they were diagnosed with cancer during the previous contract year, but did not have a Cancer Care Review completed in the 12 months following diagnosis
How to achieve the indicator
Conduct a Cancer Care Review using a structured template within 12 months following the cancer diagnosis. What will remove patients from eligibility?
- Patients who were diagnosed within the current contract year and they haven't had a cancer care review (they will also appear in next year's denominator)
- Patients for whom cancer care quality indicator care was unsuitable in the contract year
- Patients who chose not to receive cancer care in the contact year
- Patients who have not responded to at least two cancer care review invitations (at least 7 days apart) in the contract year
- Patients who were diagnosed with cancer within the last 3 months of the contract year
- Patients who were newly registered with the practice within the last 3 months of the contract year

If a patient achieves the indicator at any point during the year, any exclusions that have also been added are ignored.
