Description

The percentage of patients newly diagnosed with diabetes, on the register, in the preceding 1 April to 31 March who have a record of being referred to a structured education programme within 9 months after entry on to the diabetes register.

What do the business rules mean?

What makes a patient eligible?

Patients are eligible for the DM014 denominator if:
  1. their earliest unresolved diabetes diagnosis was after 21 months before the end of the contract year (ie: 1st July of the previous contract year)
It is worth reviewing patient records to see if they have already been referred to a structured education programme in previous years which was not appropriately coded, as this will remove them from eligibility in the current contract year

How to achieve the indicator

To meet the criteria for DM014, eligible patients must have had:

  1. referral to a diabetes structured education programme in the contract year

What will remove patients from being eligible?

  1. Patients who have been newly diagnosed with diabetes within the last 9 months of the contract year (since 1st July in current contract year) and do not have a referral to structured education programme (if referral to SEP had not already been achieved, these patients will be added back into the following contract year)
  2. Patients who have been referred to a structured education programme before the current contract year
  3. Where a code has been added stating that there isn't a structured education programme available
  4. Where diabetes structured education programme was unsuitable
  5. Patients for whom diabetes quality indicator care was unsuitable in the contract year
  6. Patients who chose not to receive diabetes quality indicator care in the contract year
  7. Patients who chose not to have diabetes structured education programme
  8. Patients who have been invited twice (at least 7 days apart) for review
  9. Patients who were diagnosed with diabetes in the last 3 months of the contract year
  10. Patients who were newly registered with the practice in the last 3 months of the contract year
If a patient achieves the indicator at any point during the year, any exclusions that have also been added are ignored.
