Dashboard Purpose
This dashboard gives you a clear view of HbA1c risk across your practice or PCN. It allows you to drill down into each risk group to see individual HbA1c targets, along with any previous personalised care adjustments. This ensures you have the information needed to monitor patient progress and provide tailored care where it’s most needed. The dashboard also highlights your progress against the HbA1c specific QOF indicators, DM020 and DM021.
Understanding the Dashboard Views
- HbA1c Risk Identification - This view shows you your the distribution of HbA1c risk across your practice.
- HbA1c QOF Indicators - This view shows you how your practice is performing against the QOF indicators, DM020 and DM020
Using Filters
Each view has filters available in the yellow bar at the top of the dashboard. These can be used to refine the information you are looking at.
Drilling down into the data
Clicking on any of the column headers will drill further into the data. An initial click will show the different HbA1c targets applicable for your patients according to QOF:
