The Investment and Impact Fund (IIF) dashboard allows you to track performance against IIF indicators across your PCN and individual practices.
It shows both current year and previous year data, helping you:
Monitor achievement during the contract year.
Identify opportunities for improvement.
Validate performance and funding after year-end.
When you open the IIF dashboard, you will first be prompted to choose the contract year:
IIF 2025–26 – live data for this year’s performance.
IIF 2024–25 – last year’s finalised results (useful for validating recorded performance and income).
The PCN Overview page summarises performance for the current IIF indicators — for example, CAN04 and HI03.
Shows % achievement for each indicator.
Colour coding indicates status:
Green – upper threshold achieved.
Orange – between lower and upper thresholds.
Red – below lower threshold.
Current Payment – income earned so far.
Missed Income – value of income lost based on current achievement.
Max Income – maximum payment possible if upper threshold reached.
Value per patient remaining – additional income for each patient required to reach the target.
Number of patients needed across the PCN to hit the upper threshold for each indicator.
Achievement % over time by month for each indicator.
Useful for spotting performance changes and evaluating the effect of interventions.
Switch to Practice Performance view to see how individual practices are performing, by clicking on the "View Practice Performance" button:
Each row represents a practice and indicator.
The bar chart shows:
Green = patients achieved.
Red = patients not achieved.
Dotted lines = lower and upper thresholds.
Patient numbers are shown as achieved / eligible.
Click on a practice and indicator to display:
Indicator name and description.
Number of patients achieved and eligible.
Specific criteria for meeting the target.
Shows practice-level achievement % over time.
Useful for tracking whether interventions are improving performance.
Focus on red/orange indicators – these have the most potential to increase income.
Check “Patients Remaining” regularly – these are your priority cases to close the gap.
Use trend data – track whether interventions deliver sustained improvements.
Validate year-end – compare current and previous year data to ensure payments match recorded achievement.
